While logged into your email, click on the Apps icon in the top right that looks like small boxes. Then click on Drive.

Click New and choose Google Forms. Also, the folder you start this in will be where it is saved by default.

If this is the first time you have used Google Forms, you will be presented with a screen similar to this. Click Get Started.

In the top left, click Untitled Form to change the name.

Type in the desired name and click on OK.

Next, click on Change Theme and then choose the style you like from the right. This is optional.

Click Edit Questions to return to the editor. Then under the responses menu, click on Choose Response Destination.

Type a name for the spreadsheet that will contain the answers others give for your form. Click Create.

At the top you will see these options. You can choose whatever you want here.

For our first question, click on the description field and type in a description. Then type in the question and any help text you may want to accompany that question. Next, click on Multiple Choice to change the question type.

For this question, I'm going to choose Text.

You can choose whether you want people to be able to skip this question or not. Click Done and then Add Item to create another question.

Type in another question with the optional help text and choose a question type. In this example, checkboxes allows you to put as many as you want by clicking the grayed out Click to Add Option. Click Done.

If you hover your mouse over a question, you will get three options on the right. First, the edit button, then duplicate question, and delete.

At the bottom of the page, you can edit the collected response message. It is not recommended to check the "Publish and show a public link to form results." If you're already done with your form, you can hit Send Form to share it with others.

If you would like to view and test your form, click on View Live Form near the top left.

Fill in your answers and click submit.

This is what your users will see after they submit their answers, with your collected answers message displayed here.

If you return to the google drive folder you started in, you will see your form and the responses sheet.
